As of yesterday most of my metadata fetching stopped working. I checked Infuse on macOS and multiple Apple TVs. Infuse on macOS and ATV is v8.1.2. I have metadata fetching turned ON and prefer embedded artwork OFF.
I submitted diagnostics and it gave me code CTDGX.
Some random movies/shows still work fine, but the majority of them don’t work.
Can you try using the Edit Metadata option on one of the affected items to reselect the correct movie/show name? Doing this should refetch the metadata and artwork for that item.
You may also check to ensure the folder(s) havent been tagged to use local metadata.

Thanks yeah, someone must have accidentally selected local metadata for the folder. Changing that back fixed it.
